First of all that " sybase mobile workflow 2.1" app in device is not compatible with sup 2.2.x so there is no use of it.
For your query, as Andrew has already mentioned your device is not able to communicate with port 5001.
Can you please share screenshots for
1. User registration for device in SCC
2. HWC settings in device
If you have installed eclipse on your laptop then connect your device to that machine and check debugging logs in DDMS.
